It has happened a couple of times. Once when I dove into a pool my top moved up from the pressure of hitting the water, but wasn't technically lost, just had to pull it down.

And another time my swimsuit bottoms almost came off because the side came untied when I hit the water while tubbing. Luckily I got it tied and fixed before the boat got back around to me. lol
