Nope only two.

the first one was multi-accounting. When I came back after my 2 year leave my mule was under "dead account holder" so I created 2 new mules actually. When I left you could have several mules. BAM 250 points and I hadn't realized the rules on mules had changed. now I get hit with a 500 point infraction for "impersonating a staff member" Fucking HI-LARIOUS

... I don't get that. 500 points for what?

I put "Assistant Admnistrator" in my profile. Nothing against that. I never CLAIMED to be anyone.

Wow. That's kind of stupid. Especially when you could have meant that you're an assistant administrator on another site or something. Maybe you should talk it out with Inso after you've calmed down?

It will be on your profile page. If you don't see it, then it means you have none.

Oh, lord. I'd better give up searching, then. I can never find anything on my profile, and that's pretty sad on my part.
