If someone needs a template, I have a screenshot of the Raw episode that might help out. :3
I can't read Japanese, but from hit-and-miss Google Translate, the top line is the person's name (Last, First), followed by the class. Under the picture there's this weird gold box thing, then (I think) it's "True Cross Order" next to that official-looking seal, and "Japan branch" underneath.
I have no idea what's after that, sadly. x_x
