My Flaaffy got taken out by a Golem's Earthquake, if that counts. xD If it doesn't count... Then I think my last death was in Halo 3. I got splattered by a Mongoose.. ;-;
Was jumping around on the landscape in Oblivion...then the game decides that I was jumping too high and decides to kill my character after he touched down...for jumping off a rock. That was barely taller than my character. >__>
I was playing SoulCaliber II with my friend, well, I was playing SoulCaliber II (she was watching me play since I kept defeating her) in survival mode as Xianghua and was killed by some dude with a REALLY big sword. >.> *isn't a SoulCaliber buff* *doesn't know the character's name*
I believe I fell through a hole in the ground in Mario Galaxy, in that galaxy you get from the Hungry Luma by the Terrace. I was a bit ambitious with my long jump. :S
PS2, Kingdom Hearts 1 Expert mode. I'm playing Olympus Coliseum and Cloud kicked my ass.
Before that, PS2, Tomb Raider Legend. Lara fell down one too many times and suffered a minor concussion. She recovered a few seconds later and got back up from where she started. : )
CADFND: I miss Soul Calibur. I have 2 and 3 but only played 3 so far.
I was playing Dante's Inferno. Had Dante fighting hoards of these flame zombie...things...and two of the seven kings until I was killed. >_>;; I can't remember exactly which level of hell I was in. It was fighting for about ten minutes straight and with me wondering when they would all just die already. @_@;;
Super Mario Bros. Wii, World 8, First level. Apparently you aren't allowed to touch the falling chunks of rock spewed from the volcano.... And right after that I discovered that you can't stand still too long before a wall of hot ash catches up with you and kills you... >.<
SO MANY RACHNII!!!! There's a bit in Mass Effect where you have to turn on this Neutron purge thing and then run through a room full of enemies to get out before the purge gets you. Run too fast,you get stuck on something and the Rachnii get you. Run too slow the purge does. It's a lose lose situation, especially on hardcore.
@carzeebear: Man I just ran through as fast as possible. I got through with very, VERY little health left but made it and I don't think I ever did it again. xD
Drilled through Father Wales only to get shot my some random splicer I didn't know was there. D=< I call cheap shot! Bioshock 2, by the way.
But the more SERIOUS death was when for whatever reason, when fighting against Todd in the Scott Pilgrim game, all of my boyfriend's friends and I got smitten with his vegan madness. D:< He's not even that harrrdddd....
So i'm still knee deep in GTA IV and still lovin' it. ;D
I was on Colony Island taking out a Most Wanted, Barry Lamora and his two friends. Barry's more rotund friend got me good with a shotgun, so afterward I headed up to the cemetery to find Nico some body armor. That done, I wanted to continue with more Most Wanted missions. My cop car had exploded, so I called 911 in the hopes of stealing a new one. The fuzz shows up, I take their car and instead of turning right, I turn left into a back alley, hit a barrel of gas and explode. ;~;

I was auto-leveling FFVII with my turbo controller and Cloud kept dying. Particularly because I probably didn't put the tape down enough or my hairtie wasn't pulling enough on the analog stick.
