Well, aside from randomly running into rocks that didn't get the memo on gravity in Mad Gallop....

...I believe the last time I died was by running into a spiked roller thing in Super Mario in 3D Land. Or jumping off a cliff, since that seems to be my death of choice lately.

I was set on fire and sent floating down the river...skyrim, why do you treat me like this?

I jumped out of a flaming sports car in the air which exploded about 1 second later sending me further into the air I hit the floor and rolled around for a bit the once I stood up a panicked bus driver ran me over, and thus, I died. The End :)

Fairy boy decided it was a better idea to just fall off the edge rather than to jump across and land safely.

Friend placed a portal while I was getting tea and I didn't realise so it killed me :(

Otherwise, I forgot how to reload my shot gun playing Resident Evil: Revelations and a zombie from beneath a bookshelf got me.

I rammed into a floating island in Mad Gallop. XD The last game on a console that I died in was Little Big Planet, where I think I was incinerated. XD Or I landed on spikes too many times. I'm not sure which.

Some guy came around a corner, and shot me. Though I got revenge by going 36/1 in that same match(calling in 3 Predators, 3 Precision Airstrikes and 3 Attack Helicopters). Of course, it was Modern Warfare 3.

Last time I died recently:
Power Rangers Samurai for the DS. The Blue Ranger ran out of health, I wasn't paying attention to his health bar and the green pants Moogers are a little tougher than him, so....

Got back-handed into a wall by a brute (Halo 3 ODST)

Fell from 45,000 feet with out realising and sank to the bottom of the Atlantic Ocean
