I apologize for not following your form to a T, but that's only beause I would like to give you as much information as I possibly can.

Character Name :: Madison Kiczys
Character Age :: Died at the age of 21
Sex :: Female
Birthday :: January 3
Western Zodiac Sign :: Capricorn
Favourite Colour :: Black
Favourite Pastime :: Watching over her daughters. Oh and tormenting Gregory.
Biography :: Due to a curse that had been placed on her while pregnant, Madison died while giving birth to her twin girls, Keori and Kalin. Afterwards, she followed her daughters around to oversee them, and was very unhappy with that she saw (they weren't treated all the worlds best). Eventually, she ran into Death (Gregory). He was trying to collect the soul of a young child whom had been brutally killed in a car crash. He was having a hard time, since the child was scared, and so Madison had stepped in. She consoled the child, calmed them down and managed to get the child to the point where they woould go with Gregory. After that, Madison became Deaths personal assistant. She collects the souls of Children and young women for him. In the process her and hm ended up bonding and forming a relationship.
Personality :: Madison is a free spirit (literally...) who really doesn't like being confined to rules. She has no problem with them, she just prefers to not have to worry about them. She is one of the kindest people you will ever meet. Unless you're doing something stupid. Madison has no problems at all using her ethereal powers to smack some sense into you. Especially if it's one of her daughters.
Appearance :: The basics are this :: She has Red hair that goes down to her waist, she usually wears it in one elastic off to the side. She has a dark gray halo and a pair of light blue angel wings. She generally wears a scarf around her neck, and an off the shoulder top with jeans. But you may change her appearance as you wish~
