Maybe it is just all from the point of Ash, and he never met any trainers with the Pokemon from other regions.

And for Glaceon it's the 'ice rock' which I just don't understand why it's not just a hunk of ice/a glacier.

Because they could argue some of them were not discovered, but the majority are far too large to go unnoticed for such a long time. Especially because they don't say the new regions were recently expanded into, like pioneering, they all seem to have been settled for at least as long as the Kanto region.

Well, okay, they do, but Ash and the gang have been traveling by foot this entire time, which is why he hasn't had contact with new areas until after he finishes exploring one area. But I can't explain why the rest of the world goes like "OH YEAH, BY THE WAY, THERE'S A NEW REGION AFTER US" without the Pokemon there being known beforehand...
