Do subscriptions only work with e-mail or private messages? I don't really understand the feature...

You can choose the option to recieve an email notification about a thread you've subscribed to having a new post, but external notices aren't necessary. You can also choose just to have a link to the thread and the new post pop up in your user-control panel, which is the area that the "My Account" link on the toolbar takes you to.
