It's probably Koudelka--bosses are tough. Also, I had no clue there was a run away command until I flung up my hands at an unbeatable gargoyle, found a walkthrough, and realized he was supposed to be unbeatable at that stage of the game.
Also, your weapons break after so many uses, adding a new level of difficulty to the game.

Trauma Center: Second Opinion (Wii) at Extreme difficulty.
TC honestly isn't that hard to finish. When you've got the hang of the Wii controls and know what surgery to perform when your patient's life is in danger, it's actually quite difficult to let your patient die.
Once you've finished the game you unlock an extra storyline that lets you try and beat the remaining levels at Extreme difficulty. Now I've played this game A LOT and managed to get XS, S or A ratings on most of my surgeries, but this is just IMPOSSIBLE.

The old NES Games they re hard to beat but I can beat almost any new game you throw at me in about 3 days. I am defiantly a video game nerd.

The toughest game I played was one I could never beat. It's called Saga Frontier 2. You play 2 different story lines. One is traditional RPG style, the other is strategy where you pit armies against each-other. The very last battle of the strategy storyline I could never win. I must have tried 50 times :(

Mine was probably Tomb Raider. I know, lame, but I don't play a whole lot of fighting, battling, tough games like that. Whenever the big mean animals came or the scary bullies/people who were going to shoot you, I made my dad play for me. lol. well, I was only like eight or nine then, so yea.

This question is slightly an oxymoron because theres a difference in a hard game from an extreemly poorly made game.
Trying to think of a game that was just too hard (Not filled with glitches and horrible controlers) I'd have to say... hmm.. lol ...
I guess Monster Hunter games with out ever playing multiplayer. Takes extreem skill right there ;) Not saying people can't, I'm saying that a brand new players, never polayed before. Good luck beating all the finnal unlockable quests. I know most of the new players give up on the first two urget quests. (Boss's)

I'd have to say the Final Fantasy series has been challenging for me.
I usually get stuck on a boss, quit for a while, and come back to it,
beat that boss... move on and get stuck again. XD

Um, the last standard boss fights in Castlevania Lament of Innocence...I beat it even though I nearly lost my sanity. I did a little video research for way to beat it and they worked. Still can't be the Forgotten one's last form though oh well, I now can try the game as the guy I unlcoked...yikes can not recall his name. ^^;

God of War 2. The puzzles and some of the boss battles. But it was still awesome. And Okami, the same.

I'd have to say Pac Man is a pretty hard game. Its easy at first but when you get to the harder levels I just want to pull my hair. Tetris is the same way.

the hardest games are the old school ones... i have never beaten Super Mario Brothers (the very first one for the NES (i think i just dated myself) ) or the original Castlevania.

i'd say final fantasy: crystal chronicles
it just dosen't end, i mean i've crossed 15 yrs in this and still can't beat the main guy!!
tales of symphonia, baten kaitos and soul calibar legends were tough too until i finally managed to make a strategy to defeat the bosses!

Final Fantasy 7 which is why after years of playing it I still can't beat it. Breath of fire 3 was also challenging mainly because of the quest with the map and having to find that place.

Hmmmmmn, toughest game......
Ridge Racer 6 is the most recent one I can think of, I got about halfway through, and then the races just became impossible to win, no matter what I did.
Also the transition from Medium to Hard on Guitar Hero, that was a killer. XD

Well, no game is too tough if you're willing to invest the time and effort.
I have to admit though, taking into account all the games I've ever played, on average I only ever completely finish maybe 10% of them. And most of the ones I have finish were the ones on older systems, like older ps1 games and stuff that came before those.
That doesn't necessarily mean the games were too "hard", but maybe that point's debatable.

My insane brother achieved that few weeks ago. He was playing on extreme and nobody saw him. It took some time though xD
But now he has this device that makes him invisble. It's a lot of fun punching a guard "Just my imagination..." The guy was punched by his imagination xD

The hardest game I've played recently was Prinny: Can I Really be the Hero? It's a side-scroller where you get a thousand lives. I went through the first two worlds and had already lost two hundred lives. The bosses are incredibly hard and making some of the jumps sucks as well. It made me far to aggravated to want to finish.

Kind of reminds me of when I played super nintendo super mario world obsessively.
The final levels are so tedious.
I had to stock up on lives from the bonus level and just go at'er with meticulous precision.
Time consuming and oddly unfulfilling at times lol...

All of the games I've ended up loving have been extremely hard at first, though normally just because of some thick headed stupidity on my part. xD; However, I do have one I want to love, but it's just so annoying and hard I haven't been able to stick with it- Orphen: Sorcerer of Scion. I'm a crazy, craaazy Orphen fan so when I saw the game for 5$ at gamestop my brain pretty much exploded. xD I took it home and proceeded to spend the rest of the day clear up until midnight trying to get far in it. I was doing pretty well... right up until the maze. In order to move on, I have to close these doors in order to make a path over them. I had all but one door figured out, and there's no way past it- the only way to close it puts you on the wrong side of the door. And only after ten minutes of running about, closing doors and sitting through the cut scenes do you figure this out. x_o I saved, went to bed and have never touched the game since.
... Well, that's not true. I touched it long enough to load it up and try and get past, only to figure out that "saving" means "starting the maze of screaming at the TV and giant iron doors all over again." Then I shut it down, put the game away and haven't touched it since. :B

Not even just the bosses, but the grunts were hard work too.
