You could use a store bought cake mix, and if you did, skip around the following quote.
If you want to make it all from scratch, then look at the quote below I got it from an old betty crocker book that I keep in my house:

Ingredients
1 cup all purpose flour
1 cup sugar
1/2 teaspoon baking soda
1/2 teaspoon salt
1/4 teaspoon baking powder
1/4 cup & 2 tablespoons water
1/4 cup plus 2 tablespoons buttermilk (I suppose regular milk would do.)
1/4 cup shortening
1 egg
1/2 teaspoon vanilla
2 ounces melted unsweetened chocolate (or, add powdered baking chocolate until it tastes right to you. Yes, this will require frequent sampling of the batter ;) )
Beat all ingredients together, and bake in a 350 degree oven for about 30 minutes.

Cupcakes are no different from cake. Just bake for about 15-18 minutes instead of 30.
You need a muffin/cupcake pan. That's unavoidable. But all you have to do to make those perfect little puffy cupcakes, is not over or under fill.
Take a tablespoon, and put about two spoons worth.
Cupcakes not coming out moist enough? Add just a little extra oil. I usually just don't measure the oil correctly to begin with...
The following recipie is something I created, and am sharing with you. Therefore, I no have to be quoting it.
Homemade Whipped Chocolate Icing
You'll need:
Whipping Cream, it comes it pints and half pints. for for a full box of batter, or about, eh, 2 dozen cupcakes (maybe more), you'll want the full pint.
Chocolate Baking Powder, about 3 tablespoons, depending on how much you like chocolate.
Sugar- It's all about how sweet you'd like it. add sugar until it's just right.
You'll also need some sort of electric mixer, but if you don't have one, all you need is a whisk, time, and elbow grease.
Whip it until it becomes thick. When you pick up the mixer or whisk, and some of the cream forms like, a pillar, then it's ready. Refridgerate until cupcakes are cool.

As for the wrappers, I usually only use store bought. But I suppose you can make your own edible chocolate ones, but once again, you need a cupcake tin.
